Introduction

Welcome to the Wyre Forest Alliance Board pages.
 
In the last 12 months I have witnessed a huge shift across Worcestershire, with the rollout of Neighbourhood Teams and a commitment to working both at scale and in a more joined up way. 

I feel proud that in the Wyre Forest we were the first to start this journey back in 2015 and we are now on the verge of delivering the type of integrated care in the local community that our patients and staff have told us they wanted.

It has not been easy but it is a testament to the dedication and hard work of alliance board members that we have been able to work through the difficulties and stay true to our original vision. Working under one roof, each Neighbourhood Team can develop inter-organisation relationships and having a single point of access will simplify the process for people looking to refer patients.

The role of General Practice is going to be crucial to the success of Neighbourhood Teams. I am really pleased that each team now has a named GP lead who will provide leadership support and work to the principles of our alliance charter. The teams have already had support from practices in leading the multidisciplinary team meetings and these have proven to be very useful and effective. Going forward there are exciting possibilities to merge the expectations of improving and extending GP access to the needs of the Neighbourhood Teams.


Over the next 12 months the teams will start to look at their local population and think of innovative ways to identify local needs and shape the delivery of care. This may involve social prescribing initiatives, use of the voluntary sector, working with care agencies and local housing officers.

I am pleased with the restructure of both the CCG and Worcestershire Health and Care NHS Trust to focus on the Alliance Boards. There is now a streamlined structure in place, which still has the Neighbourhood Teams at the very centre but it should allow the teams to focus on delivery of care, and any issues or concerns can be dealt at Alliance Board level first and then at Integrated Partnership Board level.
